Our program is designed to make a positive difference
in people’s lives by providing high-quality resources
to achieve financial independence, access to health care, education,
and training.
We help Haitian immigrant families with:
- Transportation/bus
passes;
- Scheduling medical appointments;
- Resume preparation;
- Completing applications and forms;
- Assistance with
job and educational training; and
- Referrals to other agencies

Health - HEALTHY BEGINNINGS PROGRAM
Designed to improve the pregnancy, health, and developmental
outcomes of pregnant women and infants through care coordination
services that provide education, knowledge, linkage, and the
support necessary to maximize families’ health, well-being,
and self-sufficiency.
Care Coordinators achieve this goal by helping
participants and their families receive the appropriate interventions
they need to improve situations that place them at risk.
Program Goal
Decrease the amount of low birth rates, infant mortality, and
increase the amount of healthy pregnancies in Palm Beach
County.

Health - HAITIAN FAMILY
STRENGTHENING PROGRAM
Helping families become more
self-sufficient
HEALTH
- Our goal is to educate and empower families and
individuals to adopt healthy behaviors and lifestyles.
Action
taken
- Medicaid
- Referrals to public or private agencies for
assistance.
EMPLOYMENT
- Link clients to appropriate job fairs within
city area.
- Forward client applications and/or resumes to
Human Resource managers.
EDUCATION
Encourage adult and family engagement in learning to promote
total literacy.
Action taken
- Refer clients to adult education, vocational
school, or college.

Health - BREAST
CANCER EDUCATION and Navigation Program for Haitian
Women
Goals:
- Educate Haitian women 35 years and older;
- Encourage
clinical and self breast exams; and
- Encourage yearly mammograms
Strategies
Activities:
- Outreach;
- Church presentations;
- Health Fairs;
- Referrals for Breast Clinical Exam; and
- Referral follow-ups through phone calls or home visits

Health - HIV PROGRAM
The program is designed to increase the quality of care and quality
of life for families and individuals with HIV/AIDS. We also measure
improvement to program participants as follows:
- Compliance with
medication/treatment regimens;
- Economic self-sufficiency;
- Scheduled medical appointments;
- Utilization of
health and social services;
- Reduction in high risk behaviors such as drug use and
unprotected sex;
- Sense of well-being;
- To reduce cost of care through
coordinated services which keep people with HIV/AIDS out
of the hospital; and
- Referrals to other agencies
MDEI case managers provide the following:
- Referrals
to health care;
- Application for health insurance, such as ADAP
and Medicaid;
- Hospital escort and interpretation; and
- HIV/AIDS education
and treatment adherence
